Patient Information
Whether you have established a relationship with Family Health Care, or you are visiting us for your first time, we want your experience to be the best it can possibly be. Select the appropriate section for more information.
Payment in full is expected at the time of service. We do bill most insurance carriers as a courtesy to our patients, and they require us to collect applicable co-pays prior to billing them.
We are accepting new Medicare patients, and do accept Medicaid patients if other family members with non-Medicaid coverage are currently patients.
New Patient Information
Preparing for your first visit:
- Please arrive to our office 10 minutes early
- Bring with you your drivers license and insurance card
- Bring all medication in original containers
- Have names, addresses of previous or referred doctors, so we can request pertinent medical records
- Bring completed New Patient information form packet, available in the downloadable forms section. If unable to download or print these forms, you can complete them at your appointment.
Existing Patient Information
